What places should I AVOID in Milan, the rest of Italy and in Europe in general?

I suggest where ever there is a flood or a Hollywood Actor.
Reason: with water you sink and an Actor they just treat you like crap.(unless you can prove otherwise)
As in any country, avoid the poorest parts of town.
You will not always see what they are but if the locals tell you not to go to a place, stay away.
Mostly the bad parts are not near the touristy parts of town.
Some people like tourist traps, but most will avoid them. Best buy a sensible guidebook, (I like the lonely planet series) and read up on the places you want to go.
July and August are the top of the holiday season, so plan well, and it might be worth it to book ahead or take overnight trains so you arrive early in the day and have the best chance on a good place to stay.
Just visit the tourist areas because there you can find all the services you´ll need, shopping areas, rest rooms, restaurants, hotels, nice people, hospitals, police, but, if you want to seesinght alone just contanct a travel agency or ask for a local people about interest areas, even, before trevel there you can contact some people who´s living there. Take a nice adventure.
